DSSD Show Permission to leave the signing out area without a parent or guardian.

All performers over the age of 11-17 years (Senior school aged children & above) can independently sign, themselves out if parental permission is given via this form.

I give permission for my performer/s named above to sign themselves out during DSSD dress rehearsal & DSSD Show performances P1,P2 & P3.

Option 2: I am applying to be a licensed chaperone; I will fill in the online NCC form and start the process ASAP.
Option 3: I am not applying to be a chaperone and understand I will not be able to support or go backstage with my performer.
